Background
Transfusion of procoagulant blood components (platelets, fresh frozen plasma (FFP), cryoprecipitate and fibrinogen) is common in cirrhosis, but practice varies widely between centres. Recent ISTH guidance recommends against prophylactic transfusion driven solely by abnormal laboratory values, reflecting “rebalanced haemostasis” in cirrhosis. Unnecessary transfusions should be avoided as they can cause harm and waste valuable resources. PaTRIC will describe current UK practice, compare it with guidance, and provide centre-level feedback to support quality improvement and standardisation.
Aims
Primary objective:
Measure the prevalence, patterns and indications for platelet, FFP, cryoprecipitate and fibrinogen concentrate use in hospitalised patients with cirrhosis, and compare practice with ISTH standards.
Secondary objectives:
- Evaluate clinical outcomes associated with inpatient use of procoagulant blood components in patients with cirrhosis (e.g., mortality, VTE and bleeding).
- Develop simple, effective feedback formats (including data visualisation) to improve engagement and nudge practice towards guideline alignment.
Eligibility criteria
Inclusion: All adult admissions with cirrhosis (by imaging, biopsy or clinician documentation) during your sites 2-3 weeks data collection period.
Exclusion: patients <18 years.
